Dwight W. Sievert, M.D.
Dr. Sievert has been practicing psychiatry in Fresno, CA for over 35 years. Graduate of University of Minnesota Medical School and Fresno Central Valley San Joaquin Medical Education Project residency program. He specializes in adult outpatient psychiatry focusing on diagnosis and medical management of psychiatric disorders. Dr. Sievert enjoys living and working in Fresno, CA and plans to be here for many more years.
Smadar Aviv, Ph.D.
I’m a seasoned psychologist who’s developed a unique approach to empower clients in their personal and organizational endeavors. I provide long/short term integrative/holistic psychotherapy. My services include working with adults, children, couples and families through counseling, conflict resolution, mediation and restorative justice in either divorce proceedings or marital healing. When needed, I engage in intensive up to 2-3 days project conflict resolution consults and mediation.
It is very exciting that I have been able to incorporate modern technology in coaching patients to use non defensive communication via text.
In addition to my clinical psychology training I am also certified in emotional intelligence coaching, life and executive coaching and leadership training, conflict resolution, peaceful mediation and restorative justice and Imago relationship therapy for marriage and family.
Brian Taylor, P.A.
Brian Taylor, MPAP, PA-C, is a nationally certified Physician Assistant. He completed his Bachelor of Science degree in Molecular, Cellular, and Developmental Biology from California State University, Fresno and earned his Master of Physician Assistant Practice from, Keck School of Medicine of USC. He spent time training at Didi Hirsch Mental Health Services Clinic known for being the first community-based adult outpatient clinic in Los Angeles. Using evidence based medicine, Brian will help develop a treatment plan that best fits your needs.
Andranik "Andre" Avakian, N.P
Andre Avakian is a psychiatric mental health nurse practitioner and a former clinical professor at California State University, Fresno. He specializes in Adult Psychiatry. Andre has been working in the field of psychiatry since 2012. His experience in mental health is vast and diverse. Andre has worked at three of the largest inpatient psychiatric hospitals here in the Central Valley which include Kaweah Delta Mental Health in Visalia, Community Behavioral Health Center and Exodus Recovery in Fresno, Ca. Andre works with patients ages 18 years and older and treats a broad scope of mental health conditions. Andre has Buprenorphine training and certification as a provider.

Barbara Woodward, N.P
With over 30 years of health care and mental health experience, beginning in nursing and moving on to becoming a Nurse Practitioner, Barbara seeks to understand each client and their unique experiences to provide the best level of care needed.
Born and raised in Fresno, CA, Barbara was nominated for California Nurse of the Year in 2013. She has been working in mental health and with high risk populations for greater than 23 years.
Barbara's current area of practice is addiction medicine. In her free time, she enjoys volunteering, horseback riding, backing, gardening, and exercising.
Tracy Leffler, L.M.F.T.
Crisis, major life changes, relational issues, anxiety and depression often cause individuals to seek therapy. While the difficulties that bring individuals in for therapy may seem discouraging, I believe working collaboratively with clients to create a therapeutic relationship based on trust, empathy and compassion provide an opportunity to solve problems, broaden perspectives, and motivate meaningful change.
I am privileged to work with couples, individuals, adolescents, and children. I utilize Cognitive Behavioral Therapy, Acceptance and Commitment Therapy, Solution Focus and Family Systems approaches. With a certification as a Mindfulness Mediation Teacher, I am also attentive to the ways in which responds to trauma and emotional distress and how negative stressors can affect our lives.

Joanne Lai, L.M.F.T.
Joanne was educated at the Boston Graduate School of Psychoanalysis, the Chicago School of Professional Psychology, and California State University, Fresno. She blends the traditional teachings of psychoanalysis with the modern psychology techniques such as Somatic Experiencing, Mindfulness, Art, Clinical Hypnotherapy, and Integral Breathwork Therapy into her practice. She believes that every individual is unique and requires unique treatment approach. Her integrative approaches support clients in learning about all aspects of themselves, and in developing skills for self-regulation and resilience.
Kristy Garavello, L.C.S.W.
Kristy received her Bachelor's and Master Degree at California State University Fresno.
Her clinical experience includes over 23 years of working with a variety of mental health issues and concerns, including working with victims and
perpetrators of domestic violence, divorcing families, anger issues, depression, anxiety, PTSD, trauma, self-esteem, co-dependency and relational
issues. She utilizes a collaborative approach, providing a safe nonjudgmental environment promoting inner discovery and healing. She
provides both long-term and brief therapy services with the clinical Psychodynamic focus which includes the therapeutic use of dreams.
Rachael Ensom, L.M.F.T.
Rachael received both her Bachelor’s degree and master’s degree at Fresno State University.
She’s worked with kids and adolescent population for the majority of her career in diverse populations from foster care, adoption, corrections, substance use, etc. She was in the role of Program and Clinical Director for Full Service Partnerships with County funded programs as well as a community adolescent program before making the decision to come to private practice and focus her time on connecting individually with kids and teens. She is passionate about identifying ways for youth to feel heard, viewing experiences through a trauma lens, and removing judgement and replacing with compassion as often as possible. She utilizes crafts, music, writing, and any other creative or out of the box outlets to give youth opportunities to communicate their thoughts and feelings so that they can feel heard and understood. Rachael believes it’s important first to develop a trusting relationship with the youth and then incorporate the family into the treatment with the goal of the family having the ability to communicate and “vibe” together as she slowly steps out.
Janis Sanclemente, L.M.F.T.
Janis graduated from California State University, Fresno in 2005 with her Master of Science in Marriage and Family Therapy.
Janis is a Licensed Marriage and Family Therapist that has been offering psychotherapy since 2007. She started her career working with children in the foster care system and did that for several years and moved on to working with adults with co-occurring issues for the next seven years and is now focused on working closely with: Individuals that struggle with self-esteem, stress, anxiety and depression by utilizing CBT to help them cope with the stressors of daily life. Adults struggling with issues such as co-dependency, boundary issues, emotional stress, divorce and family or relational issues.
